Abstract
Lestari Sugar Factory is one of the sugar agro-industries that requires effective supply chain management to ensure the continuity of raw material supply, production processes, and sugar distribution. This study aimed to analyze the supply chain flows, evaluate supply chain performance using the Supply Chain Operations Reference (SCOR) model and Failure Mode and Effects Analysis (FMEA), and assess supply chain sustainability using the Green Supply Chain Management (GSCM) approach. This research employed a case study method with descriptive quantitative and qualitative approaches. Data were collected through observations, interviews, documentation, and questionnaires involving seven purposively selected informants. The results showed that the supply chain consists of product flow, financial flow, and information flow, which have been implemented in an integrated manner. The SCOR assessment produced an actual performance score of 77.22, indicating a good performance category. The FMEA analysis identified Return on Capital (PA), Raw Material Quality (SR), and Production Quality (MR) as the main improvement priorities, with Risk Priority Number (RPN) values of 280, 240, and 192, respectively. Furthermore, the GSCM assessment revealed that the environmental, economic, and social dimensions each achieved a sustainability index of 86.67%, categorized as highly sustainable. Therefore, improving capital Return efficiency, raw material quality, and production quality control should become the primary focus to enhance supply chain management performance at Lestari Sugar Factory.
