Abstract
The Traditional Medicine Industry (IOT) in Indonesia continues to grow along with increasing public awareness of herbal products. PT Herba Emas Wahidatama as one of the IOT in Purbalingga, Central Java, faces the problem of defects in the packaging of Product X with an average of 7,670 pcs or 1.59% of the total monthly production, resulting in a DPMO value of 16,132 and a sigma level of 2.48. This study aims to analyze the quality control process, identify the factors causing defects, and develop recommendations for improvement. Data collection was carried out through structured interviews, direct observation in the production area, and documentation of production data for the period November 2023 - October 2024. Data analysis used descriptive methods, Six Sigma with the DMAIC (Define, Measure, Analyze, Improve, Control) approach, and Failure Mode and Effects Analysis (FMEA). The results showed that quality control includes four stages, from incoming material to finished goods. The types of defects found included bottle caps, plugs, shrink plastic, labels, and bottles. The main causes of defects are a lack of human resource accuracy and concentration, unstandardized work methods, suboptimal machine conditions, and inconsistent material quality. Recommendations for improvement include label installation training, regular machine calibration, the use of adjustable heat guns, and evaluation and improvement of packaging material suppliers. Keywords: Quality Control, Six Sigma, FMEA, DMAIC, Traditional Medicine
