Abstract
PT Wonojati Wijoyo is a company engaged in the furniture sector that uses teak wood as the basic material. It is known that at PT Wonojati Wijoyo there are many product defects in Factory 1. This is because Factory 1 is one of the production processes that is responsible for the early stages of furniture manufacturing. In this study, an analysis was conducted on the SP-15 chair because this product is the best-selling product on the market. However, PT Wonojati Wijoyo found a fairly high level of defects in the SP-15 chair product, which was 15.2%, while the company's maximum defect rate was 5%. This study aims to reduce defects in the SP-15 chair product using the Fault Tree Analysis (FTA) method and provide suggestions for improving the quality of the SP-15 chair product at PT Wonojati Wijoyo. From the results of observations, it is known that there are 3 types of defects with 11 types of basic events that cause defects in the SP-15 chair product. Then, for the most dominant type of defect, namely broken defects, there were 2726 units with a root cause probability of 0.0085 or 0.85%, while for the lowest type of defect, namely incorrect process defects, there were 835 units and a probability of 0.0034 or 0.34%. Judging from the probability results, the breakage defect has a high value, so improvements are needed to reduce defects in SP-15 chair products, compile and implement SOPs related to wood processing and install visual guides in the work area, compile standard work procedures and provide short training to all operators, organize routine technical training and operator competency tests, compile machine pressure setting guidelines according to wood type and train operators to set them, set a maintenance schedule and attach visual guides (infographics or diagrams) near the machine, so that they know the correct steps to operate the machine
