Abstract
Quality control is an important step that must be done at every stage of the production process, starting from before production begins until the final product is finished. The goal is to meet the established quality standards, improve the quality of products that do not meet the standards and maintain adequate product quality. CV. Esa Kalen Jaya is a company that produces upper shoes. Upper shoe products that have been made will be sent to the parent company of PT. Esa Kalen Jaya. Production results produced by CV. Esa Kalen Jaya has a disability of 1,542 pairs or 12.5% of the expected disability of only 9%. Uncontrolled resulting disability requires special treatment to reduce disability rates. To fix this, the FTA (fault tree analysis) method is used. FTA is used to determine the root cause of disability, factors that cause disability and provide suggestions for improvement. The result is that there are 9 disability factors including fatigue and the proposed improvement is to provide adequate rest time for employees. Keywords: quality control, fault tree analysis, upper
