Abstract

PT XYZ faces quality nonconformities in white crystal sugar production that generate defective products and quality costs. This study aimed to analyze defect levels and causes, determine improvement strategies, and assess quality cost efficiency. A descriptive and quantitative approach was used. Data were collected through observation, interviews, questionnaires, documentation, and company records for 2024–2025. The analysis applied the seven tools, Failure Mode and Effect Analysis (FMEA), and a comparison of total quality costs with sales. Total defects reached 1,667.49 tons. Sugar color defects were the largest category at 54.71%, while the control chart indicated that the process was not fully stable. FMEA placed differences in operator capability as the first priority with an RPN of 2,544, followed by aging machinery with an RPN of 2,465. Improvement strategies focused on operator competency, machine condition mapping, digital process monitoring, screen inspection, and raw material inspection. Quality costs accounted for 2.17% of sales in 2024 and were classified as efficient. The ratio increased to 2.82% in 2025, indicating inefficient quality costs.
