Abstract

Unexpected failures in reciprocating compressor pistons in the petrochemical industry not only threaten production efficiency, but also pose significant safety risks due to extreme operational loads. This study offers a preventive approach to mitigating these risks in the C-412 Compressor unit at PT Petro Oxo Nusantara by integrating Failure Mode and Effect Analysis (FMEA) risk analysis and technical verification through piston clearance measurements. Through an in-depth evaluation of severity, occurrence, and detection parameters, this study successfully isolated the Piston Ring and Ring Grooves as the most critical components with Risk Priority Numbers (RPN) of 36 and 27, respectively, which significantly exceeded the critical threshold of 20.2. The corrective interventions proved effective, validated by post-repair clearance measurement data that returned to precision in accordance with operational standards. These findings provide a new strategic foundation for maintenance management to shift from routine maintenance to risk-based maintenance to ensure sustainable system reliability.
