Abstract

Product quality is an important factor considered by consumers. PT XYZ, one of the manufacturers of orthopedic and traumatology implants. The purpose of this study was to determine the level of product defects, so that recommendations for improvement can be given. The methods used in this study were Six Sigma and Fuzzy FMEA to analyze the risk of causing defects. The results of the analysis showed that the largest defect was in the hole diameter with a total of 455 units of defects. The resulting DPMO value was 33,343, with an average sigma level of 3.33. The cause of the defect was the initial dimensions of the plate were not precise. The cause of this defect has a FRPN value of 4,64 so that repairs must be made immediately. The suggestion for improvement is to carry out regular inspection and replacement scheduling of cutting tools, as well as providing spare cutting tools that have undergone quality control.
