Abstract
This research aims to analyze risk mitigation strategies in the jumbo bag filling process at PT Petrokimia Gresik using the FMEA and FTA methods. Four significant risks have been identified, including accidents during the transfer of urea fertilizer, workplace accidents or productivity disruptions during filling, inaccuracies in weighing, and delays in the delivery of urea products. Among these four risks, there are 19 potential risk events that could hinder the filling process. The FMEA method is used to calculate the RPN to determine the priority of risk management, while FTA is employed to trace the root causes of these failures. The highest risks are related to inaccuracies in weighing and productivity disruptions during filling. As a solution, Alternative 5 was chosen, which involves automating the process using a Hopper Translator, Loadcell Weighing, Conveyor in Bag, modifications to Machine D, and PLC-based automatic control. The selection of this alternative is based on several key criteria, including: effectiveness in reducing work risks (safety), improvement in operational efficiency (time and cost), enhancement of accuracy in the filling process, ease of integration with existing systems, and positive impacts on worker satisfaction and workload. This solution resulted in enhanced automated and controlled filling system, with a process that is relatively inexpensive and quick. The research findings indicate that the integration of automation and preventive maintenance plays a crucial role in minimizing risks, enhancing workplace safety, and supporting overall operational performance.
