Abstract

This study analyzes failure risks in the material handling process at the Production Palletization Bureau of PT PAL Indonesia using the Failure Mode and Effect Analysis (FMEA) method. The main problems identified were delayed material distribution, stock data discrepancies, and incomplete pallet delivery. Based on the FMEA analysis, delayed material distribution had the highest Risk Priority Number (RPN) of 336. Improvement recommendations include implementing barcode systems, improving distribution scheduling, and applying mandatory pallet checklists to increase the effectiveness of the material handling process.
