Abstract

This study analyzes the causes of failure in the macro-test process of bogie material at PT INKA (Persero) using the Failure Mode and Effect Analysis (FMEA) method. Data were collected through field observations and interviews with Quality Assurance personnel. The analysis revealed six failure modes, with the highest Risk Priority Number (RPN) values attributed to specimen surface contamination (RPN = 252), manual polishing (RPN = 200), and the absence of pre-test Non-Destructive Testing (NDT) (RPN = 144). Recommended improvements include the implementation of cleanliness protocols, polishing automation, NDT integration, and the separation of specimen annealing cycles from bogie component processing.
