Abstract
PT Kerta Rajasa Raya is a manufacturing company engaged in the plastic packaging industry, specifically the production of plastic sacks (woven bags) and jumbo bags. Operational sustainability within the Green Supply Chain Management (GSCM) framework, which includes the plan, source, make, deliver, and return phases, is still faced with various operational and ecological obstacles that have the potential to degrade sustainability performance. PT Kerta Rajasa Raya currently has not implemented a structured risk management system to anticipate disruptions in its green supply chain activities. This study aims to identify and formulate mitigation strategies for existing vulnerabilities in the company's system. The methodology applied is the House of Risk (HOR) with the integration of Green Supply Chain Management (GSCM) principles. The House of Risk (HOR) framework is carried out through two main stages: identification of risk events and causes in the first phase, and the preparation of mitigation steps in the second phase. The research findings indicate 17 risk events triggered by 22 risk sources (risk agents). Through Pareto diagram analysis, 13 dominant risk causes were identified as priority for handling. As a solution, this study recommends 15 strategic mitigation actions to minimize the impact of these main risk sources.
