Abstract

PT Industri Kemasan Semen Gresik (IKSG) Tuban, a cement packaging manufacturing company, continues to face a high defect rate in its pasted woven bag products. This study aims to determine the level of product defects and formulate quality improvement recommendations. The Six Sigma method with a Failure Mode and Effect Analysis (FMEA) approach was applied to analyze defect risks and propose corrective actions. The analysis shows that the largest defect type is bottom path misalignment at 705,681 bags (25.37%), followed by printing defects at 697,965 bags (25.09%), sticky bags at 691,229 bags (24.85%), and burnt bags at 686,327 bags (24.67%). The average DPMO obtained is 24,788 with a sigma level of 3.46. The main cause of the most significant defect is the swollen lamination roll during the printing process, which disrupts ink absorption and print stability. The highest Risk Priority Number (RPN) of 336 was identified in printing defects, making it the primary improvement priority. Recommended actions include inspecting the lamination roll condition before printing and conducting perforation testing to ensure material readiness and improve ink adhesion. The second highest RPN of 320 was found in bottom path misalignment due to the absence of visual standards for material straightness; therefore, establishing visual position standards and machine parameter verification is recommended. The third highest RPN of 240 relates to burnt bags caused by excessive welding temperature, requiring proper temperature monitoring according to material standards and machine speed.
