Abstract
Kupang Mojokerto Eucalyptus Oil Agroindustry is an agricultural industry engaged in distilling eucalyptus leaves into eucalyptus oil products. This study aims to (1) identify the sources and types of operational risks, (2) describe the level of size of the occurrence of operational risks, and (3) formulate alternative solutions as operational risk control measures that have the potential to occur in the Kupang Mojokerto Eucalyptus Oil Agroindustry. The method used in this research is Failure Mode and Effect Analysis (FMEA) which serves to identify and analyse the occurrence of potential operational risks so that agro-industries can carry out appropriate risk control based on the risk priority number (RPN) value with a high criticality category and descriptive analysis of the results of in-depth interviews. The results showed that there were 4 sources of risk and 39 types of operational risks overall. Operational risks with a high criticality category include not maximising evaporation with RPN 415, receiving raw materials in the rainy season in wet and buddy conditions with RPN 346, dependence on certain technologies with RPN 349, and weather conditions with high rainfall intensity with RPN 339. The formulation of alternative solutions for operational risk management is carried out so that the company can control and anticipate the occurrence of risks during operational activities.
