Abstract
Rumah Sakit Islam (RSI) Sakinah Mojokerto is one of the health service providers in Mojokerto City.RSI Sakinah has utilized information technology in improving service efficiency. Every implementation of information technology must have risks, because something that provides benefits at the same time will definitely be overshadowed by risk naturally. For example, IT that provides benefits to the organization suddenly stops operating due to an incident. Therefore, RSI Sakinah Mojokerto needs a control for the identification and assessment of risk management and problem management of IT processes that occur as a result of the loss of medical record data in the information system. The purpose of this study is to obtain the capability level of the risk optimization, risk management, and problem management processes in the current and expected conditions of RSI Sakinah. This research uses COBIT 5 Assessment Process Activities as the basis for conducting assessments with the EDM03 Ensure Risk Optimization, APO12 Manage Risk, and DSS03 Manage Problems processes as the assessed processes. Furthermore, improvement recommendations will be given to RSI Sakinah. The results of the assessment show that the EDM03, APO12, and DSS03 processes are at level 1 (performed process) with an expected capability level of level 2. The proposed recommendations are to establish steps in overcoming risk management deviations if the organization fails to manage risks, complete IT-related SOPs and document every risk and problem complaint, make written reports after handling incidents that occur, and the need to create a historical directory that keeps records of every incident that occurs.
