Abstract
A construction service company that was established under the name of PT. Widya Cipta Tehnik, plays an active role in the fields of mechanical, electrical engineering and contractors. PT. Widya Cipta Tehnik focuses on the fields of Engineering, Procurement, Construction, and Service repair of industrial machines, troubleshooting for control systems, repair of electronic control modules and other control panel repairs. However, the implementation of the OHS management system in this company has received less attention from management. It is evident from work accidents that occur and there is no special division that handles K3 issues, so there is no good documentation and organization regarding the overall OHS management system. This research uses a checklist and Root Cause Analysis (RCA) method to obtain the root cause of the problem. This is done to determine the level of success of the company in the implementation of SMK3 and to share suggestions for improvement to PT. Widya Cipta Tehnik, so that it can avoid and minimize work accidents that can occur in the future.
