Abstract
Hospital wastewater contains various contaminants, including Escherichia coli and antibiotic-resistant bacteria, which may contaminate the environment and increase the risk of antibiotic resistance dissemination. Conventional wastewater treatment systems in hospitals are not specifically designed to eliminate antibiotic-resistant bacteria, indicating the need for advanced treatment technologies. This study aimed to analyze the effects of pH, FeSO₄:H₂O₂ molar ratio, and irradiation time on the removal of E. coli and changes in antibiotic resistance levels in hospital wastewater using a UV-C photo-Fenton reactor. The wastewater samples were collected from the effluent of the sedimentation unit of a hospital in Surabaya. The experimental variations consisted of pH 3 and neutral pH, FeSO₄:H₂O₂ molar ratios of 1:10, 1:15, and 1:20, and irradiation times of 0, 5, 15, and 30 minutes. The analyzed parameters included E. coli concentration based on removal percentage and log reduction, as well as antibiotic resistance determined using the disk diffusion method against ampicillin, ciprofloxacin, sulfamethoxazole, and tobramycin. The results showed that the UV-C photo-Fenton process was capable of removing E. coli under all treatment variations. The best performance was obtained at pH 3, an FeSO₄:H₂O₂ molar ratio of 1:15, and an irradiation time of 30 minutes, achieving 98.69% E. coli removal and a 1.88-log reduction. This condition also increased the sensitivity of E. coli to the tested antibiotics, as indicated by increased inhibition zone diameters. Under the optimum condition, the inhibition zone diameters reached 17 mm for ampicillin, 29 mm for ciprofloxacin, 19 mm for sulfamethoxazole, and 18 mm for tobramycin. The results indicate that pH, FeSO₄:H₂O₂ molar ratio, and irradiation time contribute to determining the effectiveness of the UV-C photo-Fenton process in removing E. coli and increasing bacterial sensitivity to antibiotics
