Abstract
PT XYZ is a fertilizer manufacturing company that seeks to maintain product quality by controlling defects in the urea bagging process. However, several bag defects are still found, causing quality issues and potential losses. This study aims to identify the defect level of urea fertilizer bags and propose improvement actions using Fault Tree Analysis (FTA) and Failure Mode and Effect Analysis (FMEA). Defect data from June 2025 to May 2026 were analyzed. FTA was used to identify root causes and calculate defect probabilities, while FMEA was applied to determine improvement priorities based on the Risk Priority Number (RPN). The results showed four defect types: torn bags (33.5%), loose stitching (26.5%), uneven stitching (21.2%), and dirty bags (18.8%). The defect probabilities obtained from FTA were 0.156%, 0.123%, 0.102%, and 0.087%, respectively, with twelve root causes identified. FMEA results indicated the highest RPN value of 256 caused by operators working in a hurry, followed by an RPN of 196 due to the lack of periodic machine calibration, an RPN of 150 due to inadequate stitching supervision, and an RPN of 100 due to low operator awareness of cleanliness. Proposed improvements include enhanced production supervision, operator training, preventive maintenance, quality inspection improvement, and housekeeping programs. These improvements are expected to reduce defect levels and improve the quality of urea fertilizer bags.
