Abstract

Hospital wastewater contains pathogenic microorganisms, antibiotic residues, and antibiotic-resistant bacteria (ARB) that have the potential to contaminate the environment and increase the spread of antibiotic resistance. Conventional treatment systems are still ineffective at removing these contaminants, making advanced treatment technologies necessary. This study aims to analyze the effect of varying types and masses of Granular Activated Carbon (GAC) as a pretreatment unit on the removal of Escherichia coli, the level of ampicillin antibiotic resistance, and turbidity, as well as to evaluate the effect of GAC adsorption on the effectiveness of UV disinfection. The study used a fixed-bed adsorption reactor combined with a continuous UV-C reactor. The study variables included the type of adsorbent (coconut shell GAC and coal-based GAC) and adsorbent masses of 225 g, 450 g, and 675 g. The parameters analyzed included Escherichia coli, the ampicillin inhibition zone diameter, and turbidity. The data were analyzed using a two-factor ANOVA. The results showed that coal-based GAC performed better than coconut shell-based GAC. The highest turbidity removal reached 84.11%, while the highest Escherichia coli removal was 42.50% with a log reduction of 0.24 at an adsorbent mass of 675 g. The combination of adsorption and UV treatment increased the ampicillin inhibition zone diameter, indicating increased bacterial sensitivity. ANOVA results showed that the type and mass of GAC had a significant effect on turbidity removal but did not have a significant effect on Escherichia coli removal or the level of antibiotic resistance. The use of GAC as a pretreatment has the potential to enhance the effectiveness of UV disinfection in hospital wastewater.
