Abstract
Posyandu Keluarga (POSGA) is an integrated basic health service program implemented by the Surabaya City Government through Mayor Regulation Number 59 of 2023 as part of community health development efforts. This research aims to analyze how health education in POSGA services is implemented as a development communication practice in Pacar Kembang Village, Surabaya City. This research employs a qualitative method using in-depth interviews with Surabaya Hebat Cadres (KSH), Puskesmas health workers, village officials, and POSGA participants. The analysis is conducted using the theory of Development Communication Multiplicity/Another, Interpersonal Communication, Social Penetration Theory, and Diffusion of Innovation and Innovation-Decision Process. The results show that POSGA implements a structured participation model an integration between formal government regulations and active participation of KSH at the local level, supported by a digital monitoring system through the Sayang Warga application. KSH proves to be a central actor functioning as a change agent through empathetic interpersonal communication that gradually builds residents' trust. The innovation adoption process among residents varies, ranging from continued adoption to re-invention that potentially leads to discontinuance. Resident resistance is the main barrier, yet it drives participatory communication innovations such as the Dasi Anting program. This research contributes to the understanding that effective health development communication requires an adaptive integration between formal structures and local participation.
