Abstract

CV. Barokah Jaya is an industry engaged in the manufacturing sector that produces high-quality rubber products. The products produced by CV. Barokah Jaya are processed rubber products, one of which is ringmat doormats. The total number of defects in ringmat doormat production is 16632 pcs. While the total production reached 164345 pcs during September 2024 to August 2025. The purpose of this study is to determine the level of defects in ringmat doormat products using the Seven Tools method and provide suggestions for improvements to reduce the level of defects in ringmat doormat products using the Failure Mode Effect Analysis method at CV. Barokah Jaya. The results of the study show that the factors that influence tear defects are found in 5 factors, namely man, material, machine, method, and environment. The highest RPN value is obtained for tear defects with a value of 729. Recommendations for improvement include training all employees, providing strict SOPs to press machine operators, providing strict SOPs to QC regarding material quality, regulating hot air flow using fans/blowers.
