Abstract

Occupational safety and health is one of the important aspects to consider in the industrial world as it can enhance productivity, promote company growth, and prevent losses caused by work accidents. There are several factors that influence occupational safety and health, one of which is human error. Therefore, this study aims to analyze and determine the value of human error probability at PT Duta Beton Mandiri using the HEART method and provide recommendations for improvement strategies using the SHERPA method. PT Duta Beton Mandiri is a company located in Pasuruan, East Java, which produces paving, concrete mixtures, and precast concrete. In carrying out production activities, the company has utilized advanced technology tools. However, work accidents can still occur due to a lack of knowledge and carelessness (human error) by employees, which can result in detrimental effects for the company. PT Duta Beton Mandiri employs 140 workers in the production department. From 2019 to 2021, there were 40 cases of work accidents, which increased by 4% compared to the previous year. The HEART method is used to evaluate the probability of human errors occurring among operators while performing their tasks. The SHERPA method is employed to identify errors caused by human skills and habits. On the other hand, the JSA method is a method used to review work methods that provide changes in work procedures in other words JSA is used to provide solutions to problems faced by companies. Based on the analysis, seven subtasks were identified as having the potential for human error, leading to work accidents. The subtask with the highest Human Error Probability (HEP) occurred during the placement of sheets under the finished paving mold, with a HEP of 0.99226, indicating a 99.22% probability of error. The subtask with the lowest HEP involved moving and arranging dry paving blocks in another area, with a HEP of 0.0202, indicating a 2.02% probability of error. From the tasks with high ordinal probability values, proposed improvement will then be given using the JSA method measures include the use of complete Personal Protective Equipment (PPE), the development of Standard Operating Procedures (SOP), and conducting daily briefings before work execution.
