Abstract

In the era of industrialization, the manufacturing sector plays a crucial role in the competitive free market. Therefore, industries in Indonesia are striving to produce products that satisfy their consumers. To provide consumer satisfaction, these industries must create high-quality products that meet predefined standards. Additionally, maintenance activities play a vital role in industrial operations by ensuring the machines' proper condition through suitable maintenance practices, thereby reducing damage and extending their lifespan. PT Petrokimia Gresik is a state-owned enterprise under PT Pupuk Indonesia Holding Company. It operates in the production of fertilizers, chemicals, construction services, and engineering. Initially, the company was under the Directorate of Basic Chemical Industry, but since 1992 it has been under the Ministry of Industry. Results and Discussion: PT Petrokimia Gresik implements a preventive maintenance system in their maintenance activities, and one of the equipment that undergoes preventive maintenance is pump 107 in the Ammonia unit. Pump 107 frequently experiences failures due to its larger size compared to other types of pumps, as well as the challenges of handling viscous or dense fluids (dark bluish). Through data collection using literature studies, previous maintenance data, and survey interviews on the types of failures, failure mechanisms, impact of failures, failure detection methods, severity levels, occurrence levels, difficulty levels of detection, and maintenance procedures, a risk assessment was conducted using the FMEA method. This assessment aimed to optimize the maintenance plan for pump 107. The method was applied in this study to identify the parameters in FMEA for five important components of pump 107: bearing, shaft, impeller, casing, and seal. In the FMEA analysis, the bearing component can experience failures that may interrupt the process due to machine vibrations. The shaft component can fail due to the use of materials that do not meet the standards, requiring pump shutdown for repairs. The impeller component can fail due to friction with the casing, leading to increased temperatures and process interruptions. The casing component can fail due to high pressure, necessitating repairs and pump shutdown. The seal component can fail due to high pressure, requiring repairs and pump shutdown. The Risk Priority Number (RPN) is the multiplication of occurrence, severity, and detection values for each failure mode. The RPN values are then ranked from highest to lowest. The highest RPN value indicates that the component requires immediate attention if a failure mode occurs. The bearing component obtained an RPN value of 84, with occurrence rated at 7, severity at 4, and detection at 3. The shaft component obtained an RPN value of 60, with occurrence rated at 6, severity at 5, and detection at 2. The impeller component obtained an RPN value of 84, with occurrence rated at 7, severity at 4, and detection at 3. The casing component obtained an RPN value of 36, with occurrence rated at 6, severity at 2, and detection at 3.
